As part of our Ask Dr. Rozina series, we recently received this thoughtful and vulnerable question:

“Can Spravato (esketamine) help with the intensity of invasive thoughts, anxiety, and depression I’m dealing with as a result of my husband’s infidelity?”

Because betrayal trauma affects many people, often quietly and on their own, we’re sharing a more detailed response here in case it helps others who may be struggling.

When Infidelity Feels Like Trauma

Infidelity isn’t just a relationship issue.

For many people, it feels like genuine trauma. When trust is broken, it doesn’t just hurt your feelings; it can shake your sense of safety, identity, and future you thought you were building.

People often describe:

  • Intrusive, looping thoughts that won’t stop replaying
  • Anxiety spikes or panic
  • Difficulty sleeping
  • Intense emotions that feel overwhelming
  • Deep sadness or feeling numb
  • Loss of focus or motivation

If this sounds familiar, please hear this first: You’re not “overreacting,” your nervous system is reacting to an emotional injury.

What Happens in the Brain After Betrayal?

Relational betrayal can activate the brain’s alarm system, especially the amygdala, the part of the brain that scans for danger. When this system becomes overactive, the nervous system shifts into threat mode and stays alert even when there isn’t a threat.

This can show up as:

  • Hypervigilance
  • Intrusive, looping thoughts (often called rumination) 
  • Emotional reactivity
  • Depressive symptoms

When symptoms are intense, something important happens: You’re not actually processing what happened anymore, instead you’re just trying to survive it. And in these moments additional support, including biological, can sometimes help calm the nervous system enough for it to heal. There are five layers of support.

  1. Self help
  2. Friend and Family
  3. Psychotherapy 
  4. Natural and conventional Medicine.
  5. Advanced Treatment including TMS and Spravato. 

So in severe or resistant symptoms, Spravato could be helpful as one of these biological support systems.  Spravato (esketemine) works differently than traditional antidepressants as it’s designed to help regulate depressive symptoms, particularly when other treatments haven’t provided enough relief.

What is Spravato?

Spravato (esketamine) is an FDA-approved medication used for:

  • Treatment-resistant depression
  • Depressive symptoms with acute suicidal ideation

Unlike traditional antidepressants that primarily target serotonin, esketamine works on the glutamate system — a pathway which plays a key role in neuroplasticity or the brain’s ability to form new connections and adapt.

In practical terms, this means it may help the brain become less stuck in repetitive distress patterns and rigid thoughts loops. It helps create space for relief and therapeutic progress.

How Can Spravato Help After Betrayal?

It is important to clarify what Spravato does and doesn’t do:

  • Spravato doesn’t erase pain.
  • It doesn’t repair a relationship.
  • It doesn’t replace therapy.

What it can do, for the right individual, is reduce the intensity of depression and intrusive looping thoughts. By calming the nervous system and lowering the overall emotional charge, it can create more internal stability.

When emotional intensity shifts from a 9 or 10 down to a 4 or 5, something becomes possible:

  • You can think more clearly.
  • You can reflect instead of react.
  • You can engage meaningfully in therapy.
  • You can begin processing the hurt rather than drowning in it.

Sometimes symptoms are so overwhelming that therapy alone doesn’t “stick.” Especially when your nervous system is in constant survival mode, it can be incredibly difficult to apply coping tools effectively. When biological treatment such as Spravato helps lower the emotional intensity, it allows a deeper healing journey to become accessible.

A Two-Layer Healing Model

Is Spravato Right for Everyone?

No.

Spravato is given carefully in a monitored medical setting and requires a comprehensive psychiatric evaluation. It’s generally reserved for individuals who meet criteria for treatment-resistant depression or who have significant depressive symptoms.

Individualized assessment matters. For some people, psychotherapy alone may be sufficient. Whereas for others medication adjustments may help.And for some, advanced treatments like Spravato or TMS may be appropriate.
